Job Opportunity: RN Hospice Case Manager (Home-Hospice Care Operations)

• Remote field work in various community settings (home, family member’s home, Assisted Living Facilities, Group Homes, Nursing Homes)

Contract Length

• 13 weeks

Patient Population

• High acuity patients with needs such as pleurx drains, PCA pumps, IV antibiotics, TPN, Foley catheters, wounds, wound VAC, chest tubes, and various drains

Caseload and Schedule

• Caseload: 22-25 patients per week
• Visits: Approximately 6 visits per day
• Shift times: Monday-Friday, 8:00 AM to 5:00 PM (must be available by phone during these hours)
• Daily huddle at 7:50 AM (can attend by phone or Teams)
• Flexible scheduling options including Tuesday-Saturday; open to working Saturdays if needed
• No weekends, no call, no holiday work required

Required Experience

• Minimum 1 year of Home Health experience required
• First-time travelers accepted if they have at least 1 year of Home Health experience
• Prior travel experience is not explicitly required

Required Certifications

• BLS (Basic Life Support)

Skills and Knowledge

• Patient and family education
• Provider communication
• Disease management
• Knowledge of troubleshooting pleurx drains, nephrostomy tubes, tracheostomies, PCA pumps, gastric suctioning (will be trained on site if needed)
• Experience with EMR systems preferred, specifically EPIC

Orientation

• Up to 2 weeks, based on comfort level
