Job Opportunity: Hospice Nurse (Home-Hospice Care Operations)

Employment Type: Contractor
Contract Length: 13 weeks

Unit Information

• Average patients per day: 105
• Minimum years of experience required: 2 years
• Preference for candidates with prior experience (first-time travelers less preferred)

Patient Population

• End stage patients
• Care settings include home, assisted living facilities (ALF), skilled nursing facilities (SNF), group homes, independent living, and hospitals for General Inpatient (GIP) care

Patient Ratios

• RN Case Manager: 7 RNs with caseloads of 14-16 patients each
• LPN: Not applicable
• CNA: 6

Required Licenses and Certifications

• Registered Nurse (RN) license
• Basic Life Support (BLS) certification
• Valid driver’s license and auto insurance

Skills and Experience

• Registered Nurse (RN) skills required
• Experience in home hospice care preferred

Technology and Equipment

• Electronic Medical Records (EMR): Epic and Rover
• IV pump usage
• Medication dispensing
• Monitors: chair and bed alarms
• Home health supplies including bag and trunk supplies

Orientation

• 1 week of unit orientation

Scheduling

• Shift: 1st shift, 8:00 AM to approximately 4:30 PM, Monday through Friday
• No weekend rotation
• No on-call duties
• No holiday work required

Requirements/Qualifications

• Valid RN license in Wisconsin
• Minimum 2 years of Case Management experience, including within the last year
• Basic Life Support (BLS) certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Hospice care experience
