Medical Surgical Nurse (LVN / LPN) - Travel/Contract

Job Overview

• Work on a 14-bed overflow med/surg telemetry unit accommodating ED boarder patients.
• Unit is a flexible space within GI Endoscopy; patient census may vary based on demand.
• Shift: Day shift, 3 shifts per week, 12 hours each (7:00 AM - 7:00 PM).
• Weekend coverage every other weekend (EOW).
• Floating required based on patient turn; may float to care for med/surg patients.
• Call requirements depend on staffing needs.
• Communication: SBAR used for ED patients; verbal report for other transfers/admissions.
• Previous charge experience preferred.
• First-time travelers accepted.
• Scrubs: Navy blue or white.

Required Qualifications

• Valid LPN / LVN license in New York State.
• Minimum 1 year of Long Term Care experience within the last year.
• Med/Surg experience.
• BLS certification issued by the American Heart Association.
• Experience with Epic electronic health record system.

Health and Compliance Requirements

• Physical exam within 1 year of start, updated annually.
• Fit mask test within 1 year, updated annually (specific acceptable mask types).
• Negative TB test or chest X-ray within 1 year; TB questionnaire accepted in lieu of annual renewal.
• Immunizations or proof of immunity required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (surface antibody positive titer, completed series, or declination)
  • Varicella
• Seasonal flu vaccination or signed declination required during flu season.
• Drug screening within 30 days of start (includes amphetamines, cocaine, fentanyl, heroin, opiates, etc.).
• Snellen eye exam within 1 year (not required annually).
• COVID vaccination copy preferred but not required.

Background and Documentation

• Government-issued photo ID required.
• 7-year criminal background check within 3 months of start (including SSN trace, national criminal, sex offender, education verification).
• EPLS/GSA/SAM and HHS/OIG checks required within 3 months and run monthly.
• Foreign equivalency education evaluation if applicable.
• Education verification required (no hard copy degrees accepted).
• Employment application and work history verification required.
• References: 2 references within 3 years required for Aya candidates.
• Facility-specific documents including photo for badge and attestation forms required.

Additional Information

• Locals within 75 miles are not accepted for this travel role.
• Minimal return-to-office restrictions preferred.
• Open to block scheduling but not guaranteed.
