Job Opportunity: RN Float - Home Health

Job Summary

• Provide nursing care in a home health setting.
• Assess patient conditions, implement care plans, and provide education and support to patients and families.
• Perform admission processes and case management duties.
• Deliver patient care following community health practice standards and organizational policies.
• Document thoroughly and communicate within an interdisciplinary team.
• Weekend rotations occur once every 6 weeks.
• Ability to travel to assigned cases is required.

Required Qualifications

• Valid Registered Nurse (RN) license in Massachusetts.
• Minimum 1 year of Home Health experience within the last year.
• Minimum 1 year of Case Management experience.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• Experience in a Home Care setting.

Required Credentials and Compliance

• Submission of a recent professional photo (color, plain background, no filters).
• Motor Vehicle Record (MVR) check within 45 days of start.
• Fit test for N95 respirator within 1 year, updated annually (no exemptions allowed).
• TB screening (skin test or blood test) within 3 months of start, updated if break >6 months.
• Documentation of MMR vaccination or immunity.
• Physician health statement within 1 year of start, updated if break >6 months.
• Criminal background check, CORI, SSN trace, national sex offender check, and exclusion checks completed within required timeframes.
• Skills checklist completed within 1 year.
• Two professional references within 4 years, including at least one supervisor.
• Resume including past 7 years of work experience with city and state.
• Education verification as required.
• Health and compliance documentation must be maintained and updated as specified.

Additional Information
