Description
Job Opportunity: HH Liaison
Position Details
- Employment Type: Contractor
- Contract Length: 13 weeks
- Shift: 8-hour Day Shift
- Hours per Week: 40
- Location: Northern area covering multiple cities (Green Bay, Oshkosh, Manitowoc, Sheboygan, Grafton); travel between sites expected (1-2 hospitals assigned, not full territory daily)
Role Overview
- Monitor referral queue and consult with patients referred from hospitals or skilled nursing facilities for hospice services.
- Provide assessments and recommendations for appropriate hospice settings when patients transition to hospice in hospital settings.
- Admit patients to hospice services within hospital settings.
- Patient census averages 2-3 patients per day.
- Patient population primarily adults with diagnoses such as heart failure, cancer, and other end-of-life conditions.
Required Qualifications
- Minimum 2 years of experience; hospice liaison experience preferred but other hospice experience acceptable.
- First-time travelers are accepted.
- Required certifications/licenses:
- BLS (Basic Life Support)
- Valid Driver’s License, Auto Insurance, and access to a vehicle.
- EPIC EMR experience required (including EPIC Hyperspace and EPIC Remote Client for charting and secure communication).
Skills and Competencies
- Proficient in symptom management for end-of-life conditions (e.g., pain, shortness of breath).
- Comfortable with end-of-life medications and educating providers on management.
- Ability to collaborate effectively with physicians.
- Knowledgeable about hospice benefits, philosophy, and different levels of hospice care (general inpatient, routine, respite).
- Understanding of various hospice service levels.
Orientation and Support
- 40 hours of unit orientation with staff.
- New Employee Orientation (NEO) provided.
- Access to supervisor/manager via phone and chat.
- Liaison team available for questions.
- Support from Hospice Medical Directors.
Work Environment and Other Information
- Dress code: Business casual or professional scrubs; white coat preferred but not required.
- Parking: Free.
- Equipment provided: Laptop and phone.
