Job Opportunity: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) – Dialysis

Location: Rochester, New York
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Details

• Shift: Day/evening shifts between 5:30 AM and 10:30 PM
• Schedule: Combination of 8-, 10-, or 12-hour shifts (e.g., 2 eight-hour and 2 twelve-hour shifts per week)
• Hours per Week: 40 hours guaranteed
• Weekend Requirement: Rotating Saturdays with similar shift hours
• Float Requirement: Must float to all other dialysis locations
• Unit Type: Outpatient dialysis (chronic and acute)
• First-time Travelers: Accepted
• Local Candidates: Must live more than 75 miles away

Required Qualifications

• Valid LPN / LVN license in New York
• Minimum 1 year of LPN / LVN experience within the last year
• Experience in an outpatient setting
• Dialysis experience
• Basic Life Support (BLS) certification issued by the American Heart Association

Health and Compliance Requirements

Candidates must complete the following health-related requirements and lab work within 5 days of contract signing:

• Physical exam (within 1 year, updated annually)
• Fit mask test (annual; acceptable masks include Halyard duckbill 1870+, 1860 Regular, 1860 Small)
• Negative TB test or chest X-ray (within 1 year) or TB questionnaire as applicable
• Immunizations or positive titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (Surface AB and AG titers required for dialysis)
  • Varicella
• Seasonal flu vaccination or signed declination (required during flu season)
• Drug screening within 30 days of start (includes amphetamines, cocaine, ecstasy, fentanyl, heroin, opiates)
• Snellen eye exam (within 1 year)
• COVID vaccination copy preferred but not required

Background and Documentation

• Government-issued photo ID (Driver’s License or State ID)
• 7-year criminal background check including SSN trace, national criminal, sex offender, and education verification
• EPLS/GSA/SAM and HHS/OIG checks within 3 months of start, updated monthly
• Foreign education equivalency evaluation if applicable
• Transcript/diploma or education verification from primary source (not required for CNA)
• State license primary source verification and NURSYS verification within 30 days of start
• Medication/pharmacology competency exam as required
• Color blindness testing (one-time)
• Competency exams as required by specialty
• Orientation safety manual acknowledgement
• Passport-style photo for badge
• Employment application and work history verification (minimum 2 past employments verified)
• References (2 within 3 years) required for direct hire candidates

Additional Notes

• Candidates must be prepared to float to other dialysis locations as needed.
• This position involves working with outpatient chronic and acute dialysis patients.
• Prior travel experience is not required.
• Local candidates must reside more than 75 miles from the facility to qualify for travel status.
