Licensed Practical Nurse (LPN) – Medical Surgical

Location: Rochester, New York
Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: 3x12 hour rotation (Day 7:00 AM–7:00 PM / Night 7:00 PM–7:00 AM)
Weekend Requirement: Every other weekend
Holiday Requirement: Available to work every other holiday, if needed

Job Description

• Provide nursing care to medical patients including those with COPD exacerbation, infections, failure to thrive, and social issues.
• Take own patient assignment with RN support in an acute care, medical-surgical, telemetry setting.
• Perform vital signs, pain assessment, intake/output monitoring, isolation precautions, physical restraints, specimen labeling.
• Administer oxygen via nasal cannula and manage feeding tubes.
• Monitor telemetry and perform 12-lead EKG.
• Administer medications (oral, IM, SQ, NG/G-tube, ear and eye drops).
• Provide IV therapy with pumps and blood glucose monitoring.
• Perform wound care basics (simple dressings, JP/Hemovac drains).
• Assist with bathing, mouth care, Foley catheter insertion, and clean catch urine collection.
• Communicate patient status changes to RN/MD.
• Floating within skillset may be required.
• Charting experience with EPIC required.

Required Qualifications

• Valid LPN / LVN license in New York.
• Minimum 2 years of LPN / LVN experience, including experience within the last year.
• Acute care experience within the last 2 years.
• Experience in medical-surgical and telemetry care settings.
• Proficiency with Epic electronic health record system.
• Basic Life Support (BLS) certification issued by the American Heart Association.

Scheduling and Work Details

• Shift rotation: 3 consecutive 12-hour shifts, alternating days and nights.
• Weekend work every other weekend.
• May be called off up to three shifts during the contract without penalty.
• Must submit requests for time off or special scheduling at time of application.
• Uniform: Navy blue and white scrub colors.

Required Documentation and Compliance

• Government-issued photo ID (Driver’s License or State ID).
• 7-year criminal background check including SSN trace, national criminal, sex offender, and education verification.
• EPLS/GSA/SAM and HHS/OIG checks within 3 months of start, updated monthly.
• Education verification from primary source (no hard copy degrees).
• Physical exam within 1 year of start, updated annually.
• Fit mask test within 1 year of start, updated annually.
• Negative TB test or chest X-ray within 1 year of start.
• Required immunizations or proof of immunity: Measles, Mumps, Rubella, Hepatitis B, Varicella.
• Drug screen within 30 days of start.
• Seasonal flu vaccination or signed declination during flu season.
• Snellen eye exam within 1 year of start.
• COVID vaccination copy preferred but not required.
• State-specific checks including NYS Nurse Aide Registry (if applicable), criminal background fingerprinting (if applicable), disciplinary action, sex offender registry, and Medicaid exclusion list checks.

Experience and Travel Requirements

• Minimum 2 years of acute care LPN experience required.
• First-time travelers accepted; prior travel experience not required.

Patient Ratios

• Day shift patient ratio approximately 1:5-6.

Additional Notes

• Candidates must complete all health-related requirements and lab work within 5 days of signing the contract.
• Floating within the unit and skillset is expected.
