Physical Therapist - Home Health (New Mexico)

Location

  • Albuquerque, NM area (within approximately 45-mile radius including surrounding communities)

Job Description

  • Provide physical therapy services within the legal scope of practice for New Mexico.
  • Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications following Medicare, state, and agency standards.
  • Develop, implement, and update individualized plans of care based on physician orders and patient needs.
  • Deliver skilled interventions and maintenance therapy programs as permitted by state scope of practice.
  • Supervise Physical Therapist Assistants (PTAs) and Home Health Aides in compliance with state practice acts.
  • Consult with physicians and interdisciplinary team members regarding treatment adjustments and patient progress.
  • Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies to promote independence and safety.
  • Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within required timeframes.
  • Apply ICD-10 coding accurately for documentation compliance.
  • Participate in Quality Assessment and Performance Improvement (QAPI) activities.
  • Provide care following infection control, safety, and risk prevention protocols.
  • Document clinical interventions, care coordination, and communication timely and accurately.
  • Participate in interdisciplinary meetings, case reviews, and continuing education.
  • Maintain compliance with HIPAA and privacy standards.
  • Work independently with minimal oversight, demonstrating initiative.
  • Use organizational technology systems for communication, documentation, and data management.
  • Provide professional and courteous service to patients, families, and partners.
  • Adapt to changing responsibilities and complete assignments with flexibility.
  • Adhere to all organizational policies and support team goals.

Required Qualifications

  • Valid Physical Therapist license in New Mexico
  • BLS certification

Work Schedule

  • Monday through Friday
