Description
Discharge Planner (Registered Nurse) - Travel/Contract
Location: Pocatello, Idaho
Shift: Day shift, 5 days per week, 8 hours per day (8:30 AM - 5:00 PM)
Contract Length: 13 weeks
Job Summary
- Responsible for medical necessity review, discharge planning, and utilization review.
- Communicates changes in patient care plans to the healthcare team.
- Maintains knowledge of community resources and referral processes.
- Anticipates patient needs and facilitates timely transitions to alternative levels of care using length of stay (LOS) data.
- Documents screenings and discharge plans consistently and promptly.
- Unit accepts first-time travelers.
- 100-mile radius rule applies; no local candidates accepted.
Required Qualifications
- Valid Registered Nurse (RN) license in Idaho
- Minimum 1 year of case management experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- BLS certification issued by American Heart Association or American Red Cross
- Experience in discharge planning
- Experience with utilization review, including InterQual criteria
Additional Notes
- Prior travel experience is not required; first-time travelers are accepted.
- Orientation and fingerprinting requirements apply for certain states (e.g., New Mexico).
- Documentation and credential verifications are strictly enforced and must be current.
This position offers a structured day shift with a focus on discharge planning and care coordination within a travel nursing contract setting.
