Description

American Traveler is seeking an experienced RN for a Discharge Planner/Case Management position requiring at least 1-2 years of experience and an Idaho or Compact RN license.

Details

Hospital-based Discharge Planner/Case Management unit
Schedule: Days, 5x8-hour shifts (8:30 AM - 5:00 PM)
13-week contract
Responsibilities include medical necessity review, discharge planning, and utilization review
Epic EMR experience preferred

Requirements

Active Idaho or Compact RN license required
1-2 years of case management or discharge planning experience required
2 professional references from a supervisor within the past 2 years required for consideration
State Board of Nursing License Verification and NURSYS Verification required for consideration

Additional Information

Identifies and communicates changes in patient plan of care to the healthcare team
Maintains working knowledge of community resources and referral processes
Anticipates patient needs using LOS data to support timely transitions to alternative levels of care
Documents initial screenings and discharge plans in the patient record consistently and in a timely manner
Candidates must reside more than 50 miles from the facility to be eligible
Former permanent staff must have been separated for at least 1 year before returning as a traveler
No more than 3 days of RTO will be auto-approved; no RTO during the first week of orientation
First-time travelers are welcome to apply
Travel pairs or groups are not accepted
