Description
American Traveler is seeking an experienced RN for a Discharge Planner/Case Management position requiring at least 1-2 years of experience and an Idaho or Compact RN license.
Details
- Hospital-based Discharge Planner/Case Management unit
- Schedule: Days, 5x8-hour shifts (8:30 AM - 5:00 PM)
- 13-week contract
- Responsibilities include medical necessity review, discharge planning, and utilization review
- Epic EMR experience preferred
Requirements
- Active Idaho or Compact RN license required
- 1-2 years of case management or discharge planning experience required
- 2 professional references from a supervisor within the past 2 years required for consideration
- State Board of Nursing License Verification and NURSYS Verification required for consideration
Additional Information
- Identifies and communicates changes in patient plan of care to the healthcare team
- Maintains working knowledge of community resources and referral processes
- Anticipates patient needs using LOS data to support timely transitions to alternative levels of care
- Documents initial screenings and discharge plans in the patient record consistently and in a timely manner
- Candidates must reside more than 50 miles from the facility to be eligible
- Former permanent staff must have been separated for at least 1 year before returning as a traveler
- No more than 3 days of RTO will be auto-approved; no RTO during the first week of orientation
- First-time travelers are welcome to apply
- Travel pairs or groups are not accepted
