Description
Physical Therapist - Home Health (Travel/Contract)
Location: Newnan, Georgia
Contract Length: 13 weeks
Schedule: Monday to Friday, 8:00 AM – 5:00 PM (40 hours/week)
Weekend Rotation: Yes, 1 weekend per month
On-Call: Yes, 1 weekend per month
Territory Coverage: Up to 75 miles from home clinic, covering Troup County (including LaGrange, West Point, Hogansville)
Average Patients per Day: 5-6 (5-10 patients assigned daily, equating to 6-8 points per day)
Productivity Expectation: 30 points per week
EMR System: Homecare Homebase (HCHB)
Type of Cases: Start of Care and Follow Ups
Required Qualifications
- Valid Physical Therapist license in Georgia
- Minimum 1 year of recent Physical Therapist experience within the last year
- Experience working with adult patients in a Home Health setting, including OASIS visits and scoring
- BLS certification
- Current driver’s license and reliable transportation
Job Details
- Home visits with periodic office meetings
- Schedule may vary by location; some positions may have a 4x10 shift Friday-Monday, but most are standard 5x8 Monday-Friday
- Local candidates accepted at the same rate
- Workload-driven need due to recruitment challenges
Compliance & Documentation Requirements
- Background checks covering last 7 years, including SS trace, county, nationwide, sex offender, MVR, and state-specific searches
- Drug screening: 10-panel required
- Physical exam within 12 months confirming good health and no communicable diseases
- Negative TB test or chest X-ray within 12 months (no pending results at start)
- Immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella as per standard requirements
- COVID-19 vaccination series or declination required
- Flu vaccination or facility-specific declination documentation required
- Education verification for applicable degree
- Two professional references required
- Work history verification for at least two most recent employers or last 7 years
- Various state-specific releases and fingerprinting may be required depending on assignment location
- Confidentiality, privacy, compliance, HIPAA, and other policy acknowledgements and quizzes must be reviewed and signed prior to start
- Personal or auto insurance policy required, must be current and under candidate’s name or list candidate as a driver
Additional Notes
- Weekend and on-call requirements vary by location; discuss during interview
- Mileage reimbursement applies only between patient homes, not commute to/from clinic
- Candidates must complete all required compliance documents and trainings before starting assignment
- Candidates must sign new supplier packets at the start of each assignment and annually if working more than one consecutive year
This position is ideal for experienced Physical Therapists with a strong background in adult home health care, comfortable with travel within a defined territory, and able to manage a caseload of home visits using Homecare Homebase EMR.
