Home Health

Day shift
5x8 hours
8:00 AM to 5:00 PM
40 hours per week

Assignment Details

Home health role with home visits and periodic office meetings
Average patient load: 5–8 patients per day
Productivity expectation: 30 points per week
Territory coverage: up to 75 miles from the home clinic
Typical travel range: 75–125 miles per week
Local candidates are accepted
No guaranteed hours
No on-call requirement
Weekend rotation may apply depending on location and willingness

Required Qualifications

Valid RN license in Pennsylvania
Home Health experience with at least 1 year of experience
Home Health experience within the last year
One completed travel assignment
BLS certification issued by the American Heart Association or American Red Cross
Associate of Science in Nursing or Associate Degree in Nursing
Experience in supervisory roles
Driver's License
Hospice experience
Experience with Homecare Homebase
Experience with OASIS

Clinical Responsibilities

Coordinate and direct home nursing care according to physician orders, agency policies, and procedures
Apply the nursing process in home health care
Supervise and evaluate home health aides and LPN/LVNs

Charting

HomeCare HomeBase proficiency required within the last 6 months
OASIS proficiency required for RNs
