Home Care Registered Nurse

Job Overview

2 years of Home Health experience
Hospice experience

Key Responsibilities

Assess patient and caregiver needs and develop a plan of care
Serve as case manager and manage caseloads and visit times
Complete OASIS documentation and other required forms accurately and on time
Implement nursing interventions and procedures
Provide patient and caregiver education
Coordinate care with other disciplines and providers
Maintain accurate nursing documentation
Coordinate safe discharges and transfers
Participate in peer review audits and case conferences
