Home-Hospice Care Operations

Overview

Contract role
Day shift, 8 hours
40 hours per week
Schedule: Monday through Friday
Average census: 6–7 patients per day

Required Qualifications

Valid RN license in Wisconsin
Associate of Science in Nursing / Associate Degree in Nursing
Case Management (3 years of experience)
Case Management experience within the last year
Home Health experience
One completed travel assignment
BLS
Driver's License

Skills / Experience Needed

Experience with wounds, IVs, ostomies, catheters, and drains

Schedule Expectations

Holiday coverage required
No weekend rotation

Work Environment

Collaborative unit culture
