Description

RN Hospice Home Care

Contract

  • 10 weeks
  • Contractor role
  • 24 hours per week
  • Weekend schedule: Saturday and Sunday, 8:00 AM to 8:00 PM

Setting

  • Home hospice role providing care in patients’ homes
  • Territory coverage includes Racine, Kenosha, and Mt. Pleasant, with occasional floating slightly outside the assigned territory

Required Qualifications

  • Valid RN license in Wisconsin
  • Hospice Palliative Care RN (2 years of experience)
  • Hospice Palliative Care RN experience within the last year
  • BLS
  • Associate of Science in Nursing / Associate Degree in Nursing
  • Experience in a(n) Home Health setting
  • Experience in a(n) Home Care setting
  • Driver's License
  • Case management experience in the home setting

Schedule Details

  • Every weekend position
  • No on-call requirement
  • Holiday coverage: 1 summer holiday and 1 winter holiday
  • RTO approval goes through the manager

Clinical Focus

  • End-of-life and terminally ill patients, including advanced cancers, end-stage COPD, and failure to thrive
  • After-hours PRN visits for symptom management, falls, and death visits

Skills Needed

  • Focused assessments and critical thinking
  • Investigating patient agitation issues
  • Foley exchange
  • Feeding tube troubleshooting
  • Wound care
  • Post-fall assessment
  • Comfort with death visits and communicating with the medical examiner or coroner
  • Administering narcotic pain medications
  • Identifying potential drug diversion in the home

Orientation and Support

  • NEO at the territory office
  • 3 shifts of orientation with an SCT nurse
  • Lead RN available 24/7
  • Other hospice SCT members available by phone

Equipment and Supplies

  • Badge, laptop, cell phone, and trunk kit provided on site

  • Navy blue scrubs
