SLP - Speech Therapist

Work Schedule

Monday to Friday, 8:00 AM to 4:00 PM
No weekends

Job Description

Perform patient assessments including swallowing, communication, and cognitive abilities.
Collaborate with patients, families, and physicians to develop and revise plans of care.
Implement treatment programs to restore function and achieve maximum functional status.
Provide discharge planning and referrals as needed.
Use age-appropriate teaching methods for patients and families and document education tools.
Maintain a safe environment by assessing social and clinical risks and following infection control measures.
Communicate effectively with physicians, nurses, and other clinical disciplines.
Comply with agency policies, documentation standards, and regulatory requirements.
Participate in team and interdisciplinary meetings, prioritize schedules, and maintain visit productivity standards.
Travel to patient homes and facilities as required using a personal vehicle.
Assist with agency growth by orienting new employees, cross-training, and accepting additional assignments.

Required Qualifications

Valid Speech Language Pathologist license in New Jersey
Experience in a Home Health setting
