Description

Job Title: Care Manager - ED

Location: State of New York

Job Summary: The Care Manager - ED is responsible for providing care coordination, utilization review, and discharge planning. This role requires a strong clinical background and the ability to work collaboratively with healthcare teams to ensure optimal patient outcomes.

Required Qualifications:

  • Five (5) years minimum of clinical experience as a Registered Nurse (RN).
  • Experience with care coordination, utilization review, and discharge planning.
  • Computer literacy.
  • Associates Degree (ASN, ADN) from an accredited nursing program.
  • RN license as a Registered Nurse in the State of New York.
  • PRI and Screen certification within three months of employment.
  • Primary source verification of license and registration of Registered Professional Nurse from NYS Department of Education (NYSED).
  • BLS certification.


Preferred Qualifications:

  • Leadership experience.
  • Bachelor's or Master's degree in Nursing.
  • Case Management Certification.
