Job Title: RN - Homecare

Job Summary:
The Registered Nurse (RN) with Homecare is a professional caregiver responsible for assessing, planning, implementing, and evaluating care for Hospice/Home Health adult and pediatric patients and their families in their home setting. The RN provides clinical care in an environment of healing and compassion while working autonomously. Care is delivered in compliance with hospice/home health policies, legal and regulatory standards, and evidence-based practices.

Required Qualifications:
 

• ASN or Nursing diploma.
• Current RN License in state of practice (verified).
• Current BLS Certification for healthcare providers.
• Current driver's license, insured and reliable transportation, and an acceptable driving record.
• Ability to communicate effectively both verbally and in writing.
• Basic computer skills.
• Ability to transfer patients independently.

Preferred Qualifications:
 

• Two years of experience working as an RN.
• Previous experience in an Acute Care, Home Health, or Hospice setting.
• Bachelor’s degree in nursing (BSN) from an accredited institution.

Additional Information:
 

• Physical requirements include the ability to lift and transfer patients, frequent computer use, and driving a vehicle.
• Frequent interactions with patients, visitors, and care providers requiring effective verbal communication.
• Manual dexterity for handling equipment and documentation.
• Ability to navigate crowded and busy environments.
• Capability to ascend and descend stairs or uneven surfaces to access patients.
