Description

Job Title: Care Manager - ED (000087)

Location: State of New York

Job Summary:
We are seeking an experienced Care Manager to join our Emergency Department team. The ideal candidate will have a strong background in care coordination, utilization review, and discharge planning. This role requires computer literacy and the ability to work collaboratively in a fast-paced environment. The Care Manager will ensure effective patient care transitions and compliance with regulatory standards.

Required Qualifications:

Minimum of five (5) years of clinical experience as a Registered Nurse (RN).
Experience in care coordination, utilization review, and discharge planning.
Associates Degree (ASN, ADN) from an accredited Nursing Program.
RN license as a Registered Nurse in the State of New York.
PRI and Screen certification within three months of employment.
Primary Source Verification of License and Registration of Registered Professional Nurse from NYS Department of Education (NYSED).
BLS certification.

Preferred Qualifications:

Leadership experience.
Bachelor’s or Master’s degree in Nursing.
Case Management Certification.
