Work Schedule:
Day/evening, you will work seven 10-hour shifts per pay period including every other weekend, and a holiday rotation required. Hours are based off the operational needs of the department. This position is hybrid with onsite work, remote work and travel required within the Madison, WI area. Your assigned work location will be the Administrative Services Building at 301 S. Westfield Rd., Madison, WI.
Pay:
Additional components of compensation include impressive shift differentials - $4/hour evening, $5/hour night and $4.25/hour weekend differential as applicable for hours worked.

Coordinate care for Home-Based Hospital Care (HBHC) patients, serving as the central point of contact through virtual visits and phone communication to ensure seamless, hospital-level care in the home.
We are seeking a Registered Nurse to:
- Partner closely with the rounding provider, equipment coordinator, admission RN, and in-home RNs/paramedics within a care team model to support daily care planning, patient monitoring, and safe transitions of care.
- Conduct virtual medication administration visits and ongoing virtual patient check-ins, providing clinical oversight and addressing changes in condition in collaboration with the interdisciplinary team.
- Participate in interdisciplinary rounds and manage post-acute referrals, discharge services, and equipment coordination to support safe discharge and continuity of care.
- Maintain thorough and timely documentation in the electronic medical record while monitoring patient progress and ensuring individualized care needs are met throughout the HBHC episode.
- This is a hybrid position combining onsite and remote work; infrequent travel to University Hospital or East Madison Hospital may be required to support patient enrollment.

- Bachelor's Degree in Nursing (BSN) Preferred
- 2 years relevant acute care or home health nursing experience Required
- Experience in care coordination, triage, or nursing leadership Preferred
- Experience in program development Preferred
- Experience in teaching self-care skills to patients and families Preferred
- Recent, relevant home health experience Preferred
- Experience using EPIC and virtual health platforms Preferred
- Experience with IV therapy and knowledge of venous-access devices and ambulatory pumps Preferred
- Licensed as a Registered Nurse (RN) in the State of WI or holds a license issued by a jurisdiction that has adopted the nurse licensure compact Required
- Current CPR/BLS Certification Required
- Relevant certification Preferred
