Job Opportunity – Registered Nurse, Case Manager – Mission Hills, CA 91345

Start Date: October 26, 2026

Location: Mission Hills, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), weekend rotation required to meet department needs. Patient ratio 1:20. No floating required.

Highlights:

Requirements:

• Active California RN license (pending licenses accepted but must be active by start date)
• Minimum 2 years of acute care hospital case manager experience required
• Experience performing care coordination and discharge planning in a hospital setting is mandatory
• Proficiency with Epic charting system required
• Experience with InterQual admission criteria, HMOs, IPAs, and arranging DME, SNF, and ambulance transport
• Knowledge of treatment options for drug/alcohol abuse, homelessness, mental health crisis, advance directives, and protective services processes
• Proficient in Microsoft applications and computerized documentation
• Must commit to working 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Two professional references covering one year within the last three years required
