Job Opportunity – Registered Nurse, Case Manager – Mission Hills, CA 91345

Start Date: October 26, 2026

Location: Mission Hills, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), weekend rotation required to meet department needs. Patient ratio 1:20. No floating required.

Highlights:

• 40 hours of comprehensive orientation provided
• Work with interdisciplinary support including social services and transportation
• Serve diverse patient populations including adolescents, adults, and geriatrics
• Professional dress code: scrubs or business attire accepted
• Epic charting system utilized

Requirements:

• Active California RN license (pending licenses accepted but must be active by start date)
• Minimum 2 years of acute care hospital case management experience performing care coordination and discharge planning
• Hospital experience is required; behavioral health-only experience does not qualify
• Proficiency with Epic charting system and computerized documentation
• Experience with InterQual admission criteria, HMOs, IPAs, and arranging DME, SNF, and ambulance transport
• Knowledge of discharge planning for complex patient situations including homelessness, mental health, substance abuse treatment, advance directives, and protective services
• Ability to independently organize and participate in family meetings and treatment plan development
• Proficient in Microsoft applications
• Two professional references covering one year within the last three years
• Commitment to work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Time off requests limited to no more than 7 days and must be requested at time of submission
