Job Opportunity – Hospice & Home Health Registered Nurse – St Albans, VT 05478

Start Date: October 8, 2026

Location: St Albans, Vermont

Schedule: Monday through Friday, 8:00 AM to 4:00 PM, with some weekend, on-call, and holiday coverage required. 37 guaranteed hours per week. Hospice coverage required when on-call.

Highlights:

• Travel nursing opportunity with a 91-day assignment through January 7, 2027
• Serve as Case Manager for assigned patients in both hospice and home health settings
• Work with an interdisciplinary team including LPNs, physical therapists, social workers, and other healthcare professionals
• Utilize high-tech nursing skills including IV maintenance, wound VAC, and complex wound care
• Provide comprehensive patient and caregiver education to support informed healthcare decisions
• Participate in monthly peer review audits and case conferences to ensure quality patient care

Requirements:

• Current Vermont Registered Nurse license in good standing
• Experience with OASIS documentation and compliance with state and federal regulations
• Proficiency in nursing process, care plan development, and case management
• Knowledge of hospice and home health nursing practices and evidence-based care
• Experience with electronic medical records systems, including McKesson processes
• Ability to work independently and manage caseloads according to productivity standards
