Job Opportunity – Home Health & Hospice Registered Nurse – St Albans, VT 05478

Start Date: November 9, 2026

Location: St Albans, Vermont

Schedule: Sunday through Thursday, 8:00 AM to 4:00 PM, 37 guaranteed hours per week. On-call rotation and holiday coverage required.

Highlights:

• 91-day travel nursing assignment providing essential home health and hospice care in a community setting
• Serve as Case Manager for assigned patients, coordinating comprehensive care plans with interdisciplinary teams
• Utilize advanced clinical skills including IV therapy, wound VAC management, and complex wound care
• Collaborate with physicians, providers, LPNs, and other healthcare professionals to deliver evidence-based patient care
• Participate in monthly peer review audits and case conferences to ensure quality outcomes

Requirements:

• Current Vermont Registered Nurse license in good standing
• Experience with OASIS documentation and home health/hospice nursing preferred
• Proficiency in McKesson electronic health record system or ability to learn quickly
• Strong assessment, care planning, and patient education skills
• Knowledge of state and federal regulations governing home health and hospice care
• Ability to work independently and manage caseload efficiently
