Case Manager

Location: Arroyo Grande, California
Employment Type: Travel
Contract Length: 13 weeks
Hours per Week: 32

Overview

This role is for a healthcare professional in case management with experience in clinical and post-acute care settings.

Required Qualifications

Valid RN license in California
Associate of Science in Nursing / Associate Degree in Nursing
Case Management (1 year of experience)
Case Management experience within the last year
Experience in a Post Acute setting
Med Surg experience
Knowledge of reimbursement benefits and requirements including Medicare, Medicaid, and managed care

Preferred Qualifications

Social Worker background
Progressive responsibility in previous job roles
Positive communication skills
Basic computer word processing ability
