Day 5x8-Hour (08:00 - 16:30), 2-year acute medical Care Manager experience performing Care Coordination/Discharge Planning, EPIC Charting, Experience with HMOs, IPAs, etc.
• Experience with Assessing patients, Care Coordination, and Discharge Planning
• Computerized documentation, required.
