🩺 Travel Registered Nurse (RN) – Care Coordinator

📍 Location: Chico, California
📅 Contract: 13 Weeks
☀️ Schedule: 8-Hour Shifts | 8:00 AM–4:30 PM | Weekdays & Weekends
⏱️ Hours: 40 Hours/Week

We're seeking an experienced Care Coordinator Registered Nurse (RN) for a 13-week travel assignment. This role focuses on coordinating patient care, utilization management, resource optimization, and collaboration with patients, families, providers, and payors throughout the episode of care.

✅ Requirements
Active California RN License
Minimum 3 years of recent Case Management / Care Coordination experience
Recent acute-care Case Management experience
Current BLS/BCLS Certification
Strong clinical assessment and care coordination skills
Experience with utilization management and resource coordination
Knowledge of admission, discharge, and observation criteria
Ability to collaborate effectively with physicians, patients, families, payors, and multidisciplinary teams

🩺 Clinical & Care Coordination Experience
Coordinate care and services for assigned patients throughout their episode of illness or treatment
Perform clinical assessment, planning, intervention, and evaluation using the nursing process
Facilitate appropriate utilization of healthcare resources
Evaluate quality and utilization cases, including admissions, observation stays, readmissions, and diagnosis-related criteria
Participate in ongoing outlier review
Coordinate patient progress and services with physicians, payors, and multidisciplinary teams
Advocate for patients and families throughout the care episode
Facilitate communication between patients, families, providers, and healthcare teams
Monitor services provided and ensure appropriate transitions of care
Coordinate discharge, transfer, or alternate levels of care
Initiate appropriate clinical pathways based on admission diagnosis
Serve as a resource for utilization management and care coordination
