Location: Sioux City, IA
Facility: UnityPoint Health
Contract: 13 Weeks
Start Date: August 31, 2026

Monday–Thursday: 8:00 AM – 4:30 PM (32-hour guarantee)
OR
Monday–Friday: 8:00 AM – 4:30 PM (40-hour guarantee)

Job Summary

Provide skilled outpatient physical therapy services to patients recovering from surgery, hospitalization, and deconditioning. Evaluate patients, develop individualized treatment plans, and work collaboratively with the interdisciplinary team to help patients regain mobility, strength, and independence.

Responsibilities
Perform comprehensive physical therapy evaluations
Develop and implement individualized treatment plans
Provide therapeutic exercise, gait training, balance training, and functional mobility interventions
Monitor patient progress and modify treatment plans as needed
Educate patients and caregivers on home exercise programs and injury prevention
Document patient care accurately in Epic
Float within the same discipline to nearby UnityPoint campuses as needed
Required Qualifications
Minimum 1 year of Physical Therapy experience
Active Iowa Physical Therapist License (Compact accepted)
Current AHA BLS Certification
Valid Driver's License
One supervisory reference at submission
Current skills checklist (within one year)
Preferred Qualifications
Completion of at least one travel assignment
Epic EMR experience
Experience treating post-operative, deconditioned, and post-hospitalization patients
Facility Highlights
