Position Summary

Applicants are required to have a minimum of 1-year certified home care experience and 1-year case management experience. This position requires the ability to travel to assigned cases.

Responsibilities

Assess patient conditions and implement care plans.
Perform admission processes in accordance with home care policies and standards.
Manage cases following community health practice guidelines.
Utilize thorough and timely electronic documentation.
Engage in interdisciplinary communications.

Unit Description

Service area includes Beverly, Marblehead, Salem, and Swampscott. Weekend rotations occur once every six weeks.
