Requirements:

Certified Home Health Care Experience
Experience with EPIC or Netsmart
OASIS Experience
Telemetry Experience
BLS Certification

Job Responsibilities:

Direct Care Duties:

Develop and revise treatment and discharge plans with patients, families, and healthcare team.

Complete and submit OASIS assessments, reassessments, and other required documents within defined time frames.

Process orders and communicate with physicians regarding patient needs and changes in condition.

Promote respect for patients' privacy, confidentiality, and property.

Implement preventive and rehabilitative strategies.

Supervise LPNs to ensure patient health and compliance with care plans.

Direct and evaluate the quality of services provided by the Home Health Agency.

Engage with patients, physicians, and other sources to ensure positive relationships and support care management philosophy.

Complete clinical records to comply with legislation and ensure seamless service delivery.

Maintain knowledge of available community-based services.

Operational and Financial Objectives:

Monitor progress, take corrective action on quality indicators, and respond to Quality Management reviews/audits.

Participate in peer review and Quality Assessment and Performance Improvement.

Maintain productivity requirements and ensure visit documentation reflects medical necessity.

Professional Growth:

Promote diversity and participate in education, training, and self-development.

Demonstrate commitment to professional growth and competency.

Responsible for achieving established Key Performance Indicators (KPIs).

Perform additional duties as requested.

Behavioral Characteristics:

Strong critical thinking, interviewing, clinical assessment, and communication skills.
Ability to exercise initiative, independent judgment, and build professional relationships.
Hold a current NYS driver's license and work independently with minimal supervision.
Adapt to change and manage diverse assignments effectively.

Preferred Experience:

One year of experience as a clinical nurse in a hospital, with nine months of home care experience preferred.
