Job Summary

We are seeking a dedicated and compassionate Home Health/Hospice Visit Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this role, you will provide comprehensive nursing care to patients in their homes or hospice settings, ensuring the delivery of high-quality, patient-centered services. Duties

Conduct thorough patient assessments, including vital signs, physical examinations, and evaluation of symptoms related to acute and chronic conditions.
Administer medications, including injections and IV infusions, following established protocols and ensuring patient safety.
Provide wound care, catheterization, tube feeding management, and other specialized procedures with precision and aseptic technique.
Educate patients and their families on disease management, medication administration, and health promotion strategies tailored to individual needs.
Document all patient interactions accurately within electronic health record (EHR) systems such as Epic or Cerner, maintaining compliance with HIPAA regulations.
Coordinate discharge planning and collaborate with case managers to ensure seamless transitions across care settings.
Assist with triage by evaluating patient symptoms over the phone or in person to determine urgency and appropriate intervention.
Support patients with complex needs such as dementia or Alzheimer's disease, memory care requirements, and behavioral health considerations.
Participate in ongoing training related to infectious disease care, pain management, palliative care, and emergency response protocols.
Work independently in home or community settings while maintaining clear communication with supervising healthcare providers.
Requirements

Valid Licensed Practical Nurse (LPN) license in good standing; additional certification in IV therapy or hospice care preferred.
Prior experience in home health nursing or hospice environments is highly desirable.
Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth.
Strong clinical skills including vital signs measurement, medication administration, catheterization, and basic life support (BLS).
Knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques.
Experience working with diverse populations including seniors, individuals with disabilities, developmental disabilities, or those requiring memory care..
Ability to perform physical examinations and assessments independently in various home settings.
Excellent communication skills for effective patient education and interdisciplinary collaboration.
