Responsibilities:Located on the Community Regional campus, Burnett Extended Care Center is our brand-new 154-bed transitional care facility. Purpose-built to serve patients transitioning from hospital to home, Burnett includes specialized units — Memory Care, Long-Term Care, and Transitional Care — that focus on quality, comfort and continuity of care. This environment offers nurses the opportunity to provide patient-centered care across diverse needs in a collaborative, post-acute setting.
- Provide direct nursing care to patients recovering from illness, surgery, or hospitalization, many with complex medical or rehabilitative needs.
- Collaborate with an interdisciplinary team to develop and implement individualized care plans that support patient goals and safe transitions of care.
- Monitor and assess patient conditions, administer medications, and document care in accordance with established policies and standards.
- Foster a supportive, team-oriented environment that promotes collaboration, open communication, and continuous learning.

Qualifications:
- Current State of California Registered Nurse license
- Current Basic Life Support for Healthcare Providers by American Heart Association
