Job Summary:
Responsible for the supervision and review of intake referrals to Agency services. Oversee the collaborative practice with management and staff to utilize proper clinical services and promote excellent patient outcomes. Responsible for the supervision of the intake department and staff.
Responsibilities:
• Reviews referrals to ensure the agency can safely meet patient care needs.
• Coordinates complex patient discharges and communicates agency capacity, patient needs, and reimbursement requirements with intake and clinical staff.
• Identifies and resolves issues to support efficient operations and positive outcomes.
• Works with leadership to improve processes, communication, and referral response times.
• Verifies insurance coverage and obtains prior authorizations when needed.
• Confirms physician authorization for home care services.
• Reviews Face-to-Face documentation for accuracy and educates referral sources as needed.
• Supervises Intake clinical and administrative staff.
• Responds to inquiries about agency services.
• Maintains and updates Intake Department policies and procedures.
• Tracks and reports referral activity and referral source data.
• Builds and maintains positive relationships with hospitals, discharge planners, and referral sources.
• Assists with onboarding, training, and mentoring of new staff and students.
• Communicates referral source concerns and opportunities for education and relationship development to leadership.
• Represents the agency professionally in all interactions.
• Participates in agency committees as assigned.
• Maintains professional knowledge through continuing education and training.
• Provides holiday and weekend coverage as needed.
• Ensure compliance with agency policies and all state and federal regulations.
• Performs other duties as assigned.
Requirements:
• NYS RN License
• 2-4 years of Home Health experience
• 2-4 years of recent Medical Surgical or Acute Care experience
• 2 years of supervisory / management experience
