Job Description:
The Inpatient Case Manager assess, plan, implement, monitor & evaluate options & services to affect an appropriate, individualized plan for hospital patient care across the continuum of care.
The Inpt CM will follow assigned patients working w/team members & Social Services to prioritize & strategize each day’s activities.
Using independent judgment & discretion, the patient must be assessed & reassessed as an ongoing process to address the patient’s broad spectrum of needs.
The Inpt CM utilizes clinical knowledge & competence, positive communication skills, problem solving & conflict resolution techniques, ability to effect change, strong skills in assessment, organization, & time management.
The CM process requires a focus on critical thinking skills, customer service skills, setting appropriate goals & measuring outcomes to effectively ensure optimal patient outcomes w/consideration to financial & health plan resources. Works under the supervision of the Accounts Payable Supervisor.
Responsible for preparing and issuing all disbursements with proper authorization in a timely manner.
Ensures the accuracy of the amounts to be paid and the proper coding.
Maintains all vendor accounts. Reviews, matches, codes and keypunches vendor invoices.
Researches problems and works with vendors and staff in professional and timely manner.
