• Education Requirements: Current RN license in the state of Wisconsin or the ability to obtain one prior to start date.
• Certifications Requirements: BLS certification, Valid Driver's License, Auto Insurance.
• Experience Requirements: Minimum of 2 years of hospice or home health care experience; experience managing end-stage patient cases preferred.
• Other Requirements: Strong interpersonal skills, ability to work independently in patients' homes, and familiarity with EMR systems such as Epic and Rover.
