Description

Seven Healthcare is seeking an experienced Case Management RN for a reputable acute‑care facility in Round Rock, Texas. This day‑shift travel assignment is ideal for a nurse skilled in discharge planning, utilization review, and interdisciplinary care coordination.

Key Responsibilities

Coordinate safe and effective patient discharge plans
Perform utilization review and ensure payer‑compliance documentation
Collaborate with physicians, nursing, therapy, and social work
Educate patients and families on care needs and community resources
Maintain accurate and timely CM documentation
Support transitions across care settings

Schedule Details

5×8‑hour day shifts
40 hours/week
13‑week travel contract

Qualifications

Active RN license (TX or compact)
Minimum 1 year recent RN Case Management or Utilization Review experience
BLS required
Strong communication, documentation, and coordination skills
