Job Requirements

• Active Registered Nurse (RN) license
• At least 1 year of nursing experience required
• Associate degree in Nursing required
• Previous home health, home care, or community-based nursing experience preferred
• Strong clinical judgment, time management, and communication skills
• Ability to work independently while maintaining a patient-centered approach
• Professional certification may be required based on assignment needs

Responsibilities

• Provide skilled nursing care in patients' homes in accordance with individualized care plans
• Perform assessments, monitor patient status, and report changes promptly
• Administer treatments and coordinate care to support optimal patient outcomes
• Educate patients and families on care plans, medications, and safety practices
• Document care accurately and maintain compliance with all clinical standards
• Collaborate with the broader care team to ensure continuity and quality of care
• Promote patient independence, comfort, and recovery in a home-based environment
