Home Health RN Job Requirements

• At least 1 year of Registered Nurse experience required
• Recent home health, homecare, or case management experience preferred
• OASIS experience required
• Active Registered Nurse (RN) license in Pennsylvania or compact eligibility, as applicable
• Strong clinical assessment, care coordination, and documentation skills
• Ability to work independently while managing a patient caseload effectively in a home care setting
• Excellent communication, patient education, and interdisciplinary collaboration skills

Home Health RN Responsibilities

• Provide skilled nursing care to patients in the home health and homecare setting
• Complete comprehensive patient assessments and develop individualized care plans
• Perform OASIS documentation accurately and in a timely manner
• Coordinate care with physicians, interdisciplinary team members, patients, and families
• Monitor patient progress and update care plans based on clinical needs
• Educate patients and caregivers on treatment plans, medications, and disease management
• Maintain accurate, thorough, and compliant clinical documentation for home health visits
• Support safe transitions of care and promote positive patient outcomes
