• Shift Duration: 8 hours
• Shift Schedule: Day shift, typically 8:00 AM to 4:30 PM
• Hours Per Week: 40
• Weekly Estimated Pay: $2,120 - $2,230

Job Requirements

• Home health nursing or hospice care experience strongly preferred
• HCHB experience or knowledge preferred
• Associate Degree in Nursing required
• Strong communication skills and excellent bedside manner
• Ability to travel daily between patient homes within the assigned service area

Responsibilities

• Provide skilled nursing care to hospice patients in their home setting
• Complete patient assessments, care planning, and ongoing case management
• Collaborate with the interdisciplinary healthcare team to coordinate care
• Travel daily between patient residences within the coverage area
• Deliver compassionate, respectful, patient-focused support to individuals and families
• Support admissions and hospice needs assessments as needed
• Maintain accurate clinical documentation and communicate patient updates effectively
