Job Requirements

• Active Registered Nurse (RN) license and appropriate documentation to practice in Massachusetts
• Minimum of 1 year of certified home care experience required
• Minimum of 1 year of case management experience required
• Strong ability to travel to patient cases as assigned throughout the Newton and Greater Boston service area
• Experience with thorough electronic documentation and interdisciplinary communication
• Ability to work independently while delivering compassionate, high-quality patient care
• Reliable, organized, and comfortable managing a dynamic daily patient schedule

Responsibilities

• Assess patient conditions and develop individualized home health care plans
• Provide ongoing education, support, and guidance to patients and their families
• Perform admission processes and case management responsibilities
• Document care accurately and in a timely manner using electronic health record systems
• Communicate effectively with the interdisciplinary care team
• Deliver care in accordance with community health standards, policies, and procedures
• Travel to assigned patient locations throughout the service area
