Overview

We are seeking a dedicated Registered Nurse (RN) to provide case management and hospice care for patients in the home setting. This is a great opportunity for an RN who enjoys autonomy, strong clinical practice, and building trusted relationships with patients and families. The assignment is 13 weeks in length with the potential for a steady full-time schedule.

• Shift Duration: 8-hour shifts
• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Weekend/On-Call: Rotation required
• Estimated Weekly Pay: $2,120 - $2,230

Registered Nurse Job Requirements

• Active Arizona RN license or compact state RN license
• Current BLS certification
• Associate degree in Nursing or higher
• At least 1 year of nursing experience preferred, ideally in homecare, hospice, home health, or case management
• Experience with HCHB and wound care preferred
• Strong communication, organization, and time-management skills
• Ability to work independently and manage a patient caseload in the home setting

Responsibilities

• Provide case management care for hospice patients in their homes
• Complete skilled nursing assessments and coordinate individualized care plans
• Support patients and families with compassionate education and guidance
• Collaborate with the interdisciplinary care team to ensure continuity of care
• Document patient visits accurately and maintain timely clinical records
• Travel within the assigned coverage area to complete home visits
• Participate in weekend and on-call rotation as scheduled
