Home Health Registered Nurse – Case Management, OASIS, EPIC – Bala Cynwyd, PA (Philadelphia Area)

We are seeking an experienced Registered Nurse (RN) for a 13-week home health and homecare assignment in Bala Cynwyd, Pennsylvania. This on-site homecare RN job is ideal for a nurse with strong case management and OASIS experience who is passionate about delivering high-quality care in the home health setting. Job Details

• Weekly Estimated Pay: $2270-$2390
• Location: Bala Cynwyd, Pennsylvania (ZIP 19004), with coverage territory in the greater Philadelphia area
• Setting: Homecare / Home Health Case Management
• Start Date: 9/30/2026
• Duration: 13-week assignment
• Schedule: 5 shifts per week
• Shift Duration: 8-hour days
• Hours: 40 hours per week
• Weekend Requirement: Every 3rd weekend required

Job Requirements

• 2+ years of recent home health or homecare nursing experience required
• OASIS experience required
• EPIC experience required
• Active Registered Nurse license
• BSN highly preferred
• Wound care experience preferred
• Strong assessment, care coordination, and case management skills

Responsibilities

• Provide skilled nursing care to patients in the home health and homecare setting
• Manage patient cases across an assigned Philadelphia area coverage territory
• Complete accurate and timely OASIS documentation
• Coordinate care plans with interdisciplinary team members, patients, and families
• Perform comprehensive patient assessments and ongoing evaluations
• Support patient education related to treatment plans, medications, and home safety
• Document care thoroughly within EPIC
• Monitor patient progress and communicate changes in condition as appropriate
