Home Health Travel Nurse Job Details

• Weekly Estimated Pay: $2,120–$2,230
• Location: Austintown, Youngstown, and surrounding Northeast Ohio communities
• Start Date: November 16, 2026
• Contract End Date: February 14, 2027
• Contract Duration: 13-week travel nursing contract
• Schedule: Monday through Friday, 8:00 a.m. to 4:30 p.m.
• Shift Duration: 8-hour shifts
• Guaranteed Hours: 36 hours per week
• On-Call Requirement: Approximately 2–3 on-call periods per week and every other weekend, including Saturday and Sunday
• Coverage Area: Mahoning, Columbiana, and Trumbull counties, with limited coverage in portions of Portage County
• Mileage: Mileage reimbursement is available between patient visits
• Dress Code: Navy blue or black scrubs

Home Health Registered Nurse Requirements

• Active Registered Nurse (RN) license in the state of Ohio
• Associate degree in nursing or equivalent minimum education
• At least 1 year of professional nursing experience; home health nursing experience is strongly preferred
• Current professional nursing certifications required for the assignment
• Strong clinical assessment, communication, organization, and electronic documentation skills
• Ability to work independently in patients’ homes while collaborating closely with an interdisciplinary care team
• Homecare Homebase charting experience is preferred
• Current clinical skills and required credentials must remain in good standing throughout the assignment

Home Health RN Responsibilities

• Provide direct skilled nursing care to patients in their homes throughout the assigned Ohio service area
• Complete comprehensive in-home patient assessments, develop and implement individualized care plans, and monitor patient progress
• Perform skilled nursing interventions and reinforce patient and caregiver education
• Maintain accurate, timely clinical documentation and Homecare Homebase charting
• Meet expected productivity standards while delivering safe, high-quality, patient-centered home health care
• Participate in on-call coverage, weekend rotations, weekly case conferences, and team meetings
• Communicate frequently and effectively with clinicians, office staff, patients, and caregivers
• Recognize changes in patient condition and promptly communicate concerns to the appropriate members of the care team
• Coordinate care and support continuity across providers, services, and care transitions
