Description
Job Overview
Responsible for coordinating patient care, discharge planning, and care transitions across the continuum in an acute care setting.
Key Requirements
- Experience:
- Strong, hands-on experience in Care Coordination (required)
- Facility Requirement:
- Prior experience with Dignity Health / CommonSpirit Health is mandatory
- License:
- Active California RN License
- Certifications:
- BLS (Basic Life Support)

Pros
I currently have been a traveler nurse for over 5 years. This is my far my favorite traveler experience. Onboarding is very easy. Jane and other communicate well. This was an issue in the beginning of my assignment but Jane quickly and nicely happen the issue. I will definitely do an assignment with this place again.
