Job Description:
Hospice Certified Nursing AssistantLocation: Chelsea to Ann Arbor
Schedule: Monday–Friday, day shift

Provide hands-on, compassionate care following each patient’s care plan
Offer comfort, dignity, and emotional support during end-of-life care
Assist patients and families in learning self-care skills
Report updates to the RN Case Manager and care team
Document care provided, changes in condition, and patient needs
Help prevent unnecessary hospitalizations and support continuity of care
High School Diploma or GED
1+ year of experience as a Nursing Assistant or Hospice Aide
Completion of a state-approved Hospice Aide training program with competency evaluation (per 42 CFR §484.36)
Hospice or home health experience preferred
Reliable transportation, valid driver’s license, and auto insurance (50% travel)
Preferred to live in the service area
Ability to lift 50–100 lbs and perform hands-on caregiving duties
