Job Description:

At Elara Caring, hospice care is about more than clinical support—it’s about dignity, compassion, and being fully present for patients and families during life’s most meaningful moments. As a Hospice CNA, you’ll play a vital role in bringing comfort, peace, and reassurance to patients in the place they call home.

Location: Grand Rapids
Schedule: Monday–Friday | Day Shift | No weekends

• Deliver hands-on, compassionate care aligned with each patient’s individualized care plan

• Support patients and families with dignity, comfort, and emotional presence during end-of-life care

• Assist with personal care and educate patients and caregivers on self-care techniques

• Communicate patient updates and changes in condition to the RN Case Manager and care team

• Accurately document care provided and patient needs

• Help prevent unnecessary hospitalizations by supporting continuity of care at home

• High School Diploma or GED

• At least 1 year of experience as a Nursing Assistant or Hospice Aide

• Completion of a state-approved Hospice Aide training program with competency evaluation (per 42 CFR §484.36)

• Hospice or home health experience preferred

• Reliable transportation, valid driver’s license, and auto insurance (approximately 50% travel)

• Ability to lift 50–100 lbs and perform hands-on caregiving duties

• Preferably resides within the service area
