Job Description:
Hospice Certified Nursing Assistant (CNA)At Elara Caring, hospice care is about more than clinical support—it’s about dignity, compassion, and being fully present for patients and families during life’s most meaningful moments. As a Hospice CNA, you’ll play a vital role in bringing comfort, peace, and reassurance to patients in the place they call home.
Location: Grand Rapids
Schedule: Monday–Friday | Day Shift | No weekends

Deliver hands-on, compassionate care aligned with each patient’s individualized care plan
Support patients and families with dignity, comfort, and emotional presence during end-of-life care
Assist with personal care and educate patients and caregivers on self-care techniques
Communicate patient updates and changes in condition to the RN Case Manager and care team
Accurately document care provided and patient needs
Help prevent unnecessary hospitalizations by supporting continuity of care at home
High School Diploma or GED
At least 1 year of experience as a Nursing Assistant or Hospice Aide
Completion of a state-approved Hospice Aide training program with competency evaluation (per 42 CFR §484.36)
Hospice or home health experience preferred
Reliable transportation, valid driver’s license, and auto insurance (approximately 50% travel)
Ability to lift 50–100 lbs and perform hands-on caregiving duties
Preferably resides within the service area
