Job Description:

Behavioral Health Clinical Team Manager (RN)

Full Time/Chicopee Office

At Elara Caring, we believe great care starts with great people. Our teams show up every day for one another and for the patients we serve — working together to provide compassionate, high-quality behavioral health care in the place patients call home.

As a Behavioral Health Clinical Team Manager, you’ll help shape a culture where clinicians feel supported, connected, and confident in their work. You’ll lead with empathy, encourage open communication, and create an environment where collaboration isn’t just encouraged — it’s how we operate. Your role is about more than managing workflows; it’s about building a team that feels valued, heard, and proud of the care they deliver.

If you’re a leader who believes a strong culture drives strong outcomes, this could be the role for you.

Why You’ll Love Working at Elara Caring
  • A supportive, team-first culture where leaders are accessible and collaboration matters

  • Autonomy paired with trust — you’re empowered to lead, not micromanaged

  • Meaningful 1:1 patient care that allows clinicians to truly connect

  • Flexible schedules that support work-life balance

  • Competitive compensation and comprehensive benefits

  • Tuition reimbursement and ongoing learning opportunities

  • 401(k) with employer match

  • Paid time off, holidays, and family & pet bereavement

  • Pet insurance

How You’ll Make an Impact
  • Create a positive, inclusive team environment where clinicians feel supported and motivated.

  • Lead and collaborate with an interdisciplinary care team to deliver coordinated behavioral health services in the home.

  • Serve as a trusted partner to clinicians, physicians, patients, and families — ensuring clear communication and shared goals.

  • Coach and support team members through regular feedback, problem-solving, and guidance.

  • Promote teamwork and accountability while ensuring high clinical standards and timely patient care.

  • Oversee plans of care with a focus on collaboration, continuity, and patient-centered outcomes.

What We’re Looking For
  • Associate’s Degree in Nursing (or related field) required

  • 2+ years of home health or behavioral health experience as a Registered Nurse

  • At least 1 year of leadership or team support experience

  • Current state Registered Nurse license

  • Comfort using Workday, HomeCare HomeBase, and MS Office tools
