Licensed Practical Nurse (LPN) – Home HealthChicopee & Surrounding Communities/ Full Time Monday through Friday 8a-5p

As a Home Health LPN, you will provide skilled nursing care under the direction of a Registered Nurse and physician while supporting patients and families throughout their healthcare journey.
Responsibilities include:
- Delivering care in accordance with physician orders, delegated nursing responsibilities, and the established Plan of Care
- Monitoring patient conditions and reporting significant changes to the RN Case Manager and healthcare team
- Assisting patients and caregivers with education and self-care techniques
- Implementing patient safety plans and utilizing available community resources
- Collaborating with the interdisciplinary care team to ensure exceptional patient outcomes
- Documenting clinical findings, nursing interventions, and medication updates accurately and timely
- Identifying opportunities to improve patient outcomes and reduce avoidable hospitalizations and emergency room visits
- Providing compassionate, professional support to patients, families, physicians, and colleagues
Graduate of an accredited Certificate, Diploma, or Associate Degree Nursing program
Current Massachusetts Licensed Practical Nurse (LPN) license
Minimum of one (1) year of clinical nursing experience
Valid driver's license, reliable transportation, and current auto insurance
Ability and willingness to travel throughout the assigned territory
Physical Requirements - Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
