Registered Nurse RN Home Health Case Manager

What You Will Do

• Manage a consistent caseload from admission through discharge
• Complete Start of Care, OASIS, recertifications, and routine visits
• Develop, implement, and adjust patient-centered care plans
• Identify clinical changes early and intervene appropriately
• Educate patients and caregivers to improve independence
• Collaborate with physicians and interdisciplinary teams
• Document efficiently using HomeCare HomeBase

What You Bring

• Active RN license
• 1+ year clinical nursing experience
• Home Health or Hospice experience preferred
• Strong assessment, organization, and time management skills
• Ability to work independently and make clinical decisions
• Reliable transportation and willingness to travel within territory

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• HomeCare HomeBase experience is a plus
