Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) – Hourly

What You’ll Do

✔ Provide direct patient care based on the Plan of Care and physician orders
✔ Support patient goals through skilled nursing visits
✔ Monitor patient condition and report changes to the RN Case Manager and care team
✔ Educate patients and families on self-care and safety
✔ Document care, medications, and updates accurately
✔ Collaborate with the RN Case Manager and clinical team
✔ Help prevent hospitalizations through proactive care

What You Need

✔ Graduate of an accredited nursing program
✔ Active LPN/LVN license
✔ Minimum 1 year of clinical experience
✔ Home health or hospice experience preferred
✔ Ability to travel within the coverage area (up to 50%)
✔ Ability to lift, push, pull, or support up to 50 pounds
✔ Valid driver’s license, auto insurance, and reliable transportation
